Clinical Pharmacists and Pharmacy Technicians

The pharmacy service aims to support patients to gain the maximum benefit from their prescribed medication, this includes medication reviews and advice from our pharmacists and technicians.
The aim of this service is to ensure that patients are taking their medication appropriately, safely, and that we review and monitor this regularly.
The pharmacy team conduct medication reviews which are in depth for patients who may be having issues with their medication or are taking large numbers of different medicines. The team will then discuss and agree a plan with the patient to manage the problem.
The pharmacy team also support the four practices by providing medicine information updates and advice to our GP's, Nurses, and administrative staff in the Practices.
The pharmacy team currently consists of 4 pharmacist and 2 pharmacy technicians.
Patients can be referred to this service via their GP or can refer themselves if they have concerns and be seen face to face or connected by a telephone call.
Health & Wellbeing Coaches

The health and wellbeing coaches work across all practices within Wansbeck PCN. The 3 coaches are assigned to individual practices and work collaboratively with the Mental health practitioners and community link workers.
The aim of the Health and Wellbeing Coaches Is to support patients to take proactive steps to improve the way they manage their physical and mental health conditions, by guiding and prompting them to change their behaviour, to empower patients so that they can make healthcare choices that matter to them.
Like the community link workers, the HWBC's spend time building trusting relationships, listening carefully to what matters and support the patients to a take action on their self-identified goals. Once the patients are identified the HWBC motivate the patient in a non-judgemental way to improve their confidence and skills, helping them become active participants in their own health and well being while treating the patient with respect and dignity.
Support is available to anyone over the age of 18 who:
- Has a long-term condition such as type 2 diabetes or chronic pulmonary disease (COPD)
- Those at risk of developing a long-term condition.
- Support for issues such as weight management
- Managing chronic pain
- Living with depression or anxiety
- Supporting with detriments of health, stress, and loneliness
- Support for making lifestyle changes.
Our team of 3 offer support over a number of sessions, face to face or in Practice or over the phone and can accept referrals made through GP practice, pharmacists, physiotherapists and Community Link workers.
Community Link Workers
The Community Link Workers (CLW) service works across all practices within Wansbeck PCN and also have a base at Hirst Welfare Centre in Ashington .
This is a non - clinical person-centred care offering a listening ear, empathy, and time for patients to unpick complex issues that are affecting their wellbeing which helps reduce health inequalities. This service offers a more holistic approach to patient's health and well being to help the individual feel empowered to focus on taking control of their lives.
The CLW's work collaboratively with other community groups and statutory services to provide practical and emotional support.
Support is available to anyone over the age of 18 who:
- Struggling with social and economic issues
- Wants more access to a more holistic approach to their health and wellbeing.
- Is not benefitting from current treatment plans.
- Has long term conditions that could benefit from additional community-based supports.
- Has low level mental health issues that may be precipitated by loneliness, isolation, debt, and grief.
Our CLW'S connect our patients to community groups and help individuals develop skills, friends, and resilience. They work with local groups and the voluntary sector to ensure accessibility and help start new groups where there are gaps in our community.
Examples of some of the services we offer at our Practices, Walk and Talk groups, Bereavement Coffee mornings and the Adult Lego Club based at Bedlington Library is proving very popular.

First contact Physiotherapy brings advanced Musculoskeletal skills into Primary care.
From neck pain to ankle sprain and all muscle and joint, tendon, bone or nerve problems in between – Patients can book an appointment through the Practice.
We have 2 practitioners within our team who can offer a detailed assessment leading to diagnosis and an outline of the available options open to patients. If necessary, the clinician can request exactly the same investigations as your GP can and make the same specialist referrals.

Mental Health Practitioners work as part of the PCN multi-disciplinary team and provide a bridge between Primary care and specialist mental health providers
Our team of 3 are responsible for delivering timely access to an integrated, high quality patient centred service that enables people with Mental health needs and or social care problems to improve their well being and functionality with the aim of preventing or reducing/ delaying the need for more specialist secondary services.
This service provides the Practices with a dedicated in house mental health resource with expert knowledge and access to further Trust services to ensure our patients are navigated at the right time to the right place. This benefits our patients by providing support in a setting which is familiar to them and as a trust it works to reduce pressures on access
them.
As well as Mental health imput the team work closely with other members of the wider team including working closely with our Community Link Workers and Mental Health and Well being coaches to provide continuity of care
Primary Care Dietitian

The Primary Care Dietitian (PCD) works as part of the multidisciplinary team within the Wansbeck PCN. Dietitians are advanced clinicians that assess, diagnose and treat dietary and nutritional problems such as overweight or obesity, diabetes, malnutrition and gastroenterology conditions, and decide on the most appropriate management pathway. We work within our clinical scope of practice to support individuals achieve their health goals. If specialist input is required, the dietitian will carry out an assessment and refer you to the appropriate specialist service.
